We collect personal health information for lawful purposes connected to providing, coordinating, documenting and managing your care and related services.

Whenever reasonably possible, personal health information is collected directly from you. We may also receive relevant information from other health-care providers involved in your care where permitted by law.

We may also disclose information where authorized or required by law.

When personal health information is disclosed, Uplift Aesthetics limits the information provided to the amount reasonably necessary for the purpose.

Your consent will be obtained where required by law. There are also circumstances under PHIA where personal health information may be used or disclosed without consent.

5. Electronic Medical Records

Uplift Aesthetics maintains patient health records using an electronic medical record (EMR) system.

Patient information stored within our EMR is accessible only to authorized individuals who require access for their role in providing or supporting your care.

We use reasonable administrative, technical and physical safeguards to protect personal health information against unauthorized access, use, disclosure, loss, alteration or destruction.

Our EMR provider, Jane, states that Canadian clinic data is stored in Canadian data centres in Montreal, Quebec and Calgary, Alberta, and that data is encrypted in transit and at rest.

Third-party integrations connected to our systems may have separate data-processing and storage practices. We take reasonable steps to ensure that service providers handling personal health information on our behalf have appropriate privacy and security protections.

6. Electronic Communication

Uplift Aesthetics may communicate with you electronically regarding appointments, treatment, follow-up and other matters related to your care.

Where appropriate and permitted, information related to your care or a referral may be transmitted using approved electronic methods, including secure electronic systems, fax or email.

We take reasonable safeguards when communicating electronically. However, electronic communication may carry inherent privacy and security risks.

Patients should avoid sending sensitive health information through personal email, text messaging or other electronic communication unless specifically requested or authorized by the clinic.

7. Clinical Photography

Photographs taken as part of your assessment or treatment may form part of your personal health record and are treated as confidential personal health information.

Separate consent will be obtained before clinical photographs are used for non-clinical purposes such as advertising, social media, our website, educational materials or promotional purposes.

A patient's consent to treatment does not automatically constitute consent to use their photographs for marketing or promotional purposes.

8. Website, Cookies and Analytics

Our website may collect non-identifying technical and usage information, including browser type, device information, IP address, pages viewed and general website activity.

We may use cookies and similar technologies to operate and improve our website, understand website usage and, where appropriate, support marketing and advertising activities.

We may use services such as Google Analytics to understand how visitors use our website and improve its functionality and performance.

Website analytics and advertising information are treated separately from information contained in your clinical health record. We do not intentionally use information from your medical record for targeted advertising.

Third-party websites, platforms and services may have their own privacy policies and practices.

9. Marketing Communications

With appropriate consent where required, we may use your contact information to send information about Uplift Aesthetics services, events, promotions and other communications that may be of interest to you.

You may unsubscribe from marketing communications at any time.

Unsubscribing from marketing communications will not prevent us from sending necessary administrative or care-related communications, such as appointment reminders or information relating to your treatment.

Uplift Aesthetics may use trusted service providers to support our operations, including providers of:

  • Electronic medical records
  • Appointment scheduling and online booking
  • Payment processing
  • Information technology and website hosting
  • Email and communication services
  • Analytics and website services

Where a third party handles personal health information on our behalf, Uplift Aesthetics will take reasonable steps to ensure that appropriate privacy, security and contractual protections are in place as required by applicable law.

Under PHIA, organizations that act as information managers for personal health information have specific obligations regarding the protection, retention and destruction of that information.

These safeguards may include:

  • Access controls and role-based access
  • Individual user accounts

No electronic system or method of transmission can be guaranteed to be completely secure.

12. Retention and Destruction

Uplift Aesthetics retains personal information and health records for as long as reasonably necessary to provide care and meet applicable legal, regulatory, professional and record-retention requirements.

Personal health information will not be destroyed simply because a patient requests its deletion where the clinic is required to retain the record.

When information is eligible for destruction, it will be securely destroyed in a manner that protects the privacy of the individual.

PHIA requires personal health information to be destroyed in a manner that protects privacy.

13. Your Privacy Rights
